[Request] Team Hall of Fame & Milestones Hall of Fame

Hi Zag,
Now that team honours and milestones are nicely populated, I think that it could be a good idea to add to the "Charts" section 2 new pages: "Team Hall of Fame" and "Milestones Hall of Fame"

Also, once done, I would rename current "Hall of Fame" page into "Player Hall of Fame"

All done, let me know any improvements.

Not really happy with the milestone design as yet. But let me know if its correctly how you think it should be.
